How to create a radio button with options displayed horizontally?

I saw the Issue Collector feature and I would like to create the same kind of radio button that is used for the "Got Feedback?" template. It uses emoticons and it is displayed horizontally.

This currently requires customizations in JIRA, which are not possible on the OnDemand platform.

Also, there's a feature request that you can check https://jira.atlassian.com/browse/JRA-9245.

This would require customization, and I believe OnDemand instances would not allow it. There is a feature request though, which you can follow: https://jira.atlassian.com/browse/JRA-9245
