How to create sign off section in jira

HI,

I want create a web fragment/section which will have list of options with checkboxes. when user click the check box (sign off), I want to capture the data of who checked the checkbox (basically current user) and when they did (current time). And, populate both next to the checkbox option (like it shown below) 

Can someone help me how to implement this?  any guidance where to start and how to do this?  is this possible in script runner?

Could you please explain why you need to use a web fragment for this and not just a standard checkbox custom field?

I am asking this because Fragments doesn't have any option to add a checkbox.

I do not want to use web fragment either. I want to develop a plugin (separate web section on JIRA Issue page) . That will let the user to check the check-box and details about who done the sign off (checked) and when they did, will populate next to the check box option. 

How can we achieve this in standard custom field?  

Is there any guidance to develop this plugin ?
