How to insert Jira ticket data to the attached Word file?

Hello,

I want to insert data from the Jira ticket fields to the attached file (Word file) in the attachment area.
The attached file is an application form and it needs some information from the ticket, like a project name, issue type, priority, etc.
The application form has special places to accept the data from the Jira ticket. I attached a pic to make it more clear what I'm meaning.
Red color words are ready to be placed by Jira's data.

Would you assist me how to insert those data please?

You can attach it automatically upon creation or after a certain transition, you can manually download, or schedule sending to an email address.
