You can archive that using Security schemes, in the configurations of your project.

Basically, you can configure security levels for your items. Once a level is set to an item, only the people (in your case, group) specified in the level will be able to see it. You can only set one level per issue, and as you mentioned, when you want someone to be able to see all issues, just include them in all groups, or include de user in all security levels.

Yes. The way to address this would be to set up issue level (or in the new terminology: work item level security schemes). They allow you to specify at the work item level who can access it.
