How to send Jira sprint information to ServiceNow

When a sprint is completed in Jira we would like to create a ServiceNow change request automatically.

The change request in ServiceNow should contain a link to the related sprint in Jira and a list of stories\defects completed in the sprint.

Does anybody know if this is possible with out of the box functionality (REST API) or can be developed somehow?

 It is also helpful to know if this can be achieved with third party integration tooling.
