I need to send a survey form directly to the reporter, whenever a Jira issue is closed.
I am using Jira Software (not Service Desk).
Is this possible to achieve?

What does the Form need to do?
If this is similar to the JSM "Satisfaction Form", then there isn't a specific feature for this in Jira Software. But there might be an option using Screens/Workflow Transitions, depending on your specific need.
