Inline Images Always Broken in JIRA

I often post images in my comments.  For UI work especially, I find this very helpful for explaining a problem or a solution.  For the longest time I have not been able to directly copy/paste in images into comments.  When I paste it just shows up as a broken image.  The workaround for this was to paste in the area up above, where it says "Drop files to attach," then use the "attachment" (paper clip) button on the comment toolbar to insert the named image directly.  That would always work, then I just had to switch to "Text" mode and remove the annoying "|thumbnail" text to get it to show full size.

Now even THAT is not working.  I can attach the image as a thumbnail no problem, just as before, but if I monkey with the text at all the image breaks.  I tried removing "|thumbnail" and I tried replacing it with "|width=400" but nothing works.  All I get is breakage.

So for now all I can have is a thumbnail, which is not the end of the world, but not ideal.  Any thoughts on what's causing this and how to fix it?
