Hello!

Is it possible to do a link to Confluence in a custom filed in Jira and to have a possibility to follow it?

You can use the "links" to insert links to pages.  This has the advantage of updating automatically if the confluence page moves or changes (if the Confluence has an Application link with Jira)

Or you could use a custom "url field" - these expect you to paste urls in and presents them as links.  If you're going to do that, I'd recommend using the short links to Confluence pages so that page moves and renames do not break the links in the field.
