We want to simplify a request form as much as possible.
For instance, the form would have a question like "Add another address?" which, when clicked, would show fields for another address. Is that possible?
Or, is it possible to have a multi-step request form, kind of like a request workflow where they need to complete a multi-step logic-conditional process before the request can become an issue?
In fact, this "Ask a question" form seems to do some of the things I'm talking about
Looking to achieve the same we have JIRA server. We want to limit the number of fields on a screen but would like specific fields to show based on specific custom fields that are answered. How to achieve this using JIRA server? Is there a user friendly add-on versus using any scripting?

I'm afraid that's not possible using JIRA Cloud. You might be able to achieve something like that customizing JIRA Server, but for Cloud, I don't believe we have anything like that available.
