Is there a documentation for Personal Issue Templates for JIRA

Hello,

I saw a new jira plugin at Marketplace (https://marketplace.atlassian.com/plugins/com.atlassian.jira.plugins.personal-issue-templates).

I installed this plugin, but there is no documentation available therefore I don't know how to use it. Does somebody know how to use this plugin?

It is fully documented and tested.
