Is there a way to find out which external sites the app I'm using in my instance has access to?

Just as the title says, I am trying to find a way to determine which external sites the application in my instance has access to and what information is being shared with these external sites.

Some apps list a partial list of their data egress.  These lists are always partial, because once the data went to a data egress site, it can be copied further.

Your best bet is to look into the Terms of service of the apps you consider, and possibly ask the vendor.
