Is there a way to query the number of times a task status changes from say 'Code-Review' to 'Test'?

I want to report on the number of times a task fails CodeReview and resent to development or testing. Is there a JQL query or way to extract this information per task/bug or issue type? thanks

you can have a look at our add-on Status History PRO and one of reports, Changing Status History Report, which will show you not only issues, but also their statuses and users working on them. More details you will find here.

You can not get this info with JQL. JQL only return issues. 

You can use a plugin like Time In Status:
