Issue Collector - Customize Got Feedback

For the issue selector,  the template of Got Feedback, I want to make the name and email fields required.  I have already read some documentation that you need to create a custom template to do so, but then I cannot add the options that are in the Got Feedback template.  

How can I either make those fields required, or replace them with ones that are?
