Issue Collector: Is there a way to add more fields in the issue collector form on a confluence page

I have limited fields available for the issue type I am creating in JIRA via the issue collector. I have hosted the intake form on a confluence page.

I have used a HTML code to add preloaded questions in the description fields but would like to make it more user friendly.

Is there a way to add more fields just on the form on the confluence page and then collate all the infor into the description field once the user hits submits and jira issue is created in the project

You can use the custom template and include additional fields for your issue collector. Thus, you wouldn't need to come up with a custom HTML to handle that. Have a look at the following links to help you configure additional fields.
