We'd like hiring managers to leverage JIRA to request account access for new employees and contractors, and existing team members. We would like the hiring manager to create an issue by checking off, in the issue creation step, the accounts to which they're requesting access, to dynamically create subtasks for each account requested. Some of these subtasks will have to be dependent (email address setup before account X can be set up).
Does anyone have suggestions for 1. including check-boxes in the issue wizard and 2. creating a dynamic and interdependent workflow?

Im actually in the process of building this Exact request in my own Cloud instance.

The addon above allows you to place conditions and post functions which can do exactly what you asking for.
For our own form:
User opens a specific issue type (Security Request Form at our company)
User checks systems/software/ specific department sets ups from a drop down we populated via custom fields.
Tier 1 tech then reviews the submitted issue and ensures its valid
Tier 1 tech transitions the issue to the next step in the workflow; the transition has post functions that check the fields for values and use IF/Then statements to create and link tasks to be sent out to the different teasm who provide access.
