We have a system where managers at our office can grant certain access'es, but all requests for this comes trough the IT Helpdesk. So, no matter if they themselves choose to grant access or leave it to IT, they still need to approve the users request.
But I wonder, in Jira Servicedesk, is there any way to give a extra choice when approving? Like normally you can either approve or decline, but would it be possible to have something like "(Approved, I will fix), (Approved, IT will fix), (Declined)"?

No there is not. The approval in JSM is very basic. To work around this you may need to build the approval options instead to the workflow process (for example after approval, the approved moves the issue to the next status signalling what the next action will be).
