My company has 500 jira licenses, however only 20 people will use the Swanly , do I buy the swanly license for 500 people or can I buy only 20 and inform which accounts they will have?

No, you can't do that. Apps are billed based on the number of licensed users of your Jira instance.
To add what @Trudy Claspill has already said, I will add that the Atlassian Marketplace partners who develop Atlassian add-ons have no choice in this. It is a condition of doing business on the Atlassian Marketplace. There are a myriad of good reasons why this is so.
That said, the Marketplace vendors understand that many of our products are not used by every Jira (or Confluence) user. And, we price our products accordingly.
In other words, if we could sell you a license for 20 users (out of 500) the price would most likely be roughly the same (assuming the normal ratio of users for the product in question is 4% of the total numbers of Jira users).
The question you have to ask yourself, is do you see enough value in product X to pay the price you see on their marketplace listing?
I'm sure that's not what you wanted to hear, but that's the way things work.

Jira's pricing structure has to be the stupidest model I've ever seen, my workplace and many others like mine have to share Jira with different departments.
Why on earth would Atlassian think that a company would opt for that plugin and pay 5 - 20 times as much for something when only 1 department would use it?
Not only does it prevent companies from adding plugins but it also takes away adoption for plugin developers.
I would have by now added quite a few plugins if it wasn't for this ridiculous pricing model.
There are other options out there that aren't within the Jira eco-system, what a wasted opportunity.

I would recommend that you look at standing up a separate Jira instance just for those 20 users. This would allow you to use Swanly and only pay for those 20 users. This technique comes with its own set of problems and might not be ideal, but if you are on a budget, moving those 20 users to their own Jira might be a plausible solution for you. Otherwise, like everyone above stated, you have to pay for all 500 users.

How do I determine my Cloud pricing?
Apps are billed based on the number of users in your Atlassian product. Jira Cloud apps are priced based on the maximum users of the Jira products on your instance. For example, if you have Jira Software (50 users) and Jira Service Management (10 agents) on the same instance, you pay the 50-user price for apps.
Note: While this app has features specific to Jira Service Management and Jira Software, the app is technically available across the whole Jira instance. Therefore the above guidelines for licensing across maximum users still apply.The pricing structure for cloud apps is as follows:
- Monthly subscriptions with up to 10 Atlassian product users are billed at a flat rate price.
- Monthly subscriptions with more than 10 users are billed per user, starting at the first user.
- Monthly subscriptions with more than 100 users are billed per user. The table above provides example pricing at various user levels.
Annual subscriptions may offer a discount depending on the number of users purchased.
