My javaScript Code is not loading on clicking on a weblink?

Hi,
After clicking on a web item a javascript cal should happen and it should display the alert message but the script file itself is not loading.here is the code.
Below i have added jquery file and added a sampleReport.js i.e my file which contains an alert message should be displayed.
This is my sampleReport.js code
Can anyone suggest me ,wat i need to add to load and execute my js code ?
