Need to reference multiple custom fields based on a parent field

Jira v 8.20.1 Data Center

We are creating a project for users to submit software requests (COTS, FOSS, etc).  However they can have multiple entries for one Product type.  For each 1 of those entries we need certain information for each type:

For Example:

A user could submit a request that contains requests for 15 Foss Products and for each of those Products we need certain information like Product Name, Product Version, License Type, License restriction, etc to be filled out.  I know I could create a table, but is there someway  to have a form that supports multiple entries and fields without creating a table

Have you look at proforma for JIRA ?

You cannot do that with JIRA vanilla.
