No "Create Issue" link in Crucible

I would like to create issue in Jira directly from Crucible review, but I don't have link "Create Issue" shown in documentation.

Jira and Crucible are connected as described in documentations, and all other requests are met.
I tried linking review to a task (it was linked to a sub-task at first), but still nothing.

The Issue Creation from Crucible functionality has been vastly improved in the latest version of Crucible. You no longer need to configure anything, apart from an Application Link to at least one JIRA instance.

You will need to upgrade to at least a 3.x version. The latest version is 3.1.3.

You might want to create a support request - its very difficult to diagnose this problem without enough information (which you may not want to post on a public site).
