Outlook Integration for Jira Disappearing

I am using Outlook 365 with Jira Integration successfully on 2 computers.  The 3rd computer is using the same version of Outlook 365 but I cannot enable the Outlook Integration for Jira Com Add in.  I check off the checkbox on the COM Add-ins controls and click OK, but the Jira integration tools is not available.  I go back into the COM Add-ins controls and the Checkbox is now unchecked.  

   It is as if Outlook is ignoring my selection to include the Outlook Integraiton for Jira Add-IN.  Does anbody have any suggestions?

I have uninstalled and reinstalled the add-in.
