QTest attachments are uploaded into Jira by another operator but my name appears

When another operator has uploaded QTest attachments into Jira, it is appearing as though I have done the upload even though in the comments field it mentions the other operators name as the person who did the upload.

 

How can I remedy this as it has happened multiple times?
