1. I guess, that the approval process depends on process peculiarities, so it`s better to build different ones.
2. The approver will be notified when the Form is submitted. The approver will get a Jira and email notification.
3. The approvers can find the list of tickets assigned to them, but only after form submission.
4. After Approve/Reject assigned users will get an email notification about approval status.
For this case, try out Business Process Manager, developed by my team.
Hi, @rbalamu2. This app has only 22 installs so I think it's unlikely you will be lucky enough to find a user here on the Atlassian Community who's willing to respond.
IMO, you'll have much better luck using the Email vendor or Submit a support request links found on the Atlassian Marketplace page that you shared above. 🙂
