Any recommendations for a trend chart that shows tickets moving from status 'x' to status 'y' during a certain period?

I am looking for a trend chart in JIRA (or a widget for Confluence) that can show a count of tickets moving from one status to another each week. I currently use a JQL to pull this information on a weekly/monthly basis. Wondering if anyone knows of a report that can can be configured to pull this information for me over a certain duration (year), so I can see the trend of tickets for each week??
