Can I add a custom field to the version form?

The default fields on the version form are name, description and release date. Is it possible to add additional custom fields?

No it's not possible.. Custom fields can only be applied to issues, not even projects.

If you want to add extra information to a version you'll have to build a plugin to do so. It might be that the JIRA metadata plugin has done this already for you. It depends a bit on your use-case I guess.
