Creating task based on workflow transition

Good day,

Please advise on how do we go about creating a task or subtask, based on a workflow transition.

So everytime the workflow moves from i.e. 'In Progress' to 'Forum Review', we want for a 'Forum Review' task/subtask to be created and automatically assigned to a nominated user (say the project lead)...

Please advise.

If you are on cloud, you can use a web hook

Find or write a post-function addon that does "create subtask".

There are actually a couple in the marketplace already, and I'd start with them, they're good.  If they don't work for you, consider the script runner addon.
