Custom Issue (New Access Request) With A Dynamic Form

Hello,

I'm hoping someone can assist with this issue.  I am wanting to create a custom task/issue for new hires "New Hire Access" that has dropdown menu that allows the user to pick Marketing, Sales, Engineering and so on.  

Each dropdown has different options for their respective areas (Engineering will need Admin access, Sales needs Sales system access and so on).   So the idea is, the hiring manager can create an for their area, select from a custom list of systems needing to be accessed and assign it out. 

I have created cascading sheets but this gives the option to choose only one system. 

Can someone point me towards some resources that will teach me to do this?

Does anyone have a cloud add in that will help accomplish this?

You would need to get an add on to accomplish what you would like to do.  We ended up using multiple checklists to accomplish a similar access request in our service desk.
