Custom Velocity Chart

I need to create a custom velocity chart that can show specific dates and go back at least a year, I also want to show story points completed each sprint per user. 

Can i do this in Jira or can anyone suggest an add in that i can use?

I am not sure if you can specify dates for velocity chart in Jira. However, I do have an add-on suggestion for this. This is an add-on that I have created - https://marketplace.atlassian.com/apps/1218888/simple-custom-reports-charts-for-jira

In this app, overview report can show story points assigned and completed per sprint. Also, you can add a user as a filter to narrow this data to each user.
