Different screen depending on one first field

Hi everybody,

context: we created a "Cost Request" issue type, used for internal purchase orders and cost tracking in general.
This issue type has its own screens, screens scheme, etc.
With a global loop we've created an Action, so that any developer working on a Jira issue can quickly click "send cost request" from the Action menu. This opens a customized screen with all the fields that need to be filled in, and finally creates the "Cost Request" issue.

Problem: I'd like to have one field appearing first, asking the user to choose between option 1 or 2.
I'd like a different screen to open, with different fields depending on the chosen option.
e.g. If I choose option 1 I have to fill in the fields x,y,z, with option 2 x,j,k.

Would it be possible to have something like this? If so, how?

Alex, you're probably looking to build a form. You can use "sections" in the form and set conditions to hide or show those sections depending on the selection of a previous field. Hi Bailey, thanks for your reply.

I have checked the forms but I think I would need to have Jira Service Management to be able to use them as explained in the documentation you linked.

We don't use it, and the only forms I can find are in Issue settings/Forms but are then called Data Connections.
It seems to allow me to connect to an external form (e.g. Google Form, even thought I tried it and it doesn't work because it doesn't return the result in Json), but not to create one like in the docs.

Would you have any other suggestions...? Thanks!
