I created a screen for creation of a ticket with some custom fields but when I create a new ticket I don't see these fields?
You can see I've set the fields on my screen and its assigned to the issuetypes. When I created a story you can see that the custom fields aren't showing.

If you're a project admin, when you create a new issue type, you should be able to see the "Configure Fields" button like the below image:
Click that and a window should pop up and show all of the fields that you can add to the ticket. Your custom fields should be there and un-checked. Check them and they should appear on tickets that you have.
