How can you trigger a resolution screen when Defect is resolved?

We want to add a resolution screen so that when developers transition defect to Resolved the user must enter the Resolution and Root Cause fields.

I don't see where/how this is accomplished via Workflow (triggers, validators, post functions).

This is actually defined on the transition, not on the status.
