How do I add custom fields to a form in a team managed project?

How do I add custom fields from an issue type that has custom fields to a Form?

When I try to create a Form, there are default fields availalbe.  Since this is a team-managed project, I'm guessing this is a limitation to the company-managed projects where I can modify Issue Type Schemes with custom fields that can be added to a Form.

I'm hoping the community may have run into this and has a suggestion.
