How to add default comment with pre-populated value during a transition without using plugins.

For example, when user change the status, a comment screen should appear with pre-populated content in it. User should further add his comment.

If you are comfortable editing your workflow, you can create and assign a screen to the desired transition.  Adding the field to the new screen will let your users add the comment and update your issue.
