Setup a screen that maps fields to correspond to your template.(including specifying if an information is required or not).
Add the newly setup screen to the create issue operation of the screen scheme of the project.
If you are not creating the ticket directly from JIRA then do let me know how they are being created so I can advise.

My understanding is you have a list of items you want to be present in all tickets that are created in your JIRA. - correct me if I misunderstood here.

I didn't get how I can create a custom field and then add it to new screen, so I could create issues from this template. Could you provide a link explaining that?

I created a custom field with checkbox in it. I would like to do an issue type with this customizable checkbox. So when I choose this issue, The checkbox is already there.
I need:
1) understand the dependency between screens - fields - issues.
Do I need to customize fields first, then implement that in the screen, associate the screen with the project? Where is the issue type in this case?
2) add the dates to the sub-tasks in my issue, when I am creating them via fields?
3) or fields are not issued for issues ? And I need to create issues first and then implement them in fields?

You still have not told us what you mean by "template"?
Or do you just mean "a default value for a field when I create an issue"? Defaults can be set under the field context configuration (admin -> custom fields -> your field -> configure)
