Good Idea is to Export few issues from JIRA first and see how values are stored for different fields. Then prepare your file in the same manner/format.

Are you importing a csv? how big is it? Is it malformed?

Have a look at the detailed log for a detailed error message. A Fatal error is blanket diagnosis.
