This question is in reference to Atlassian Documentation: How to add a comment during a transition
We use this "comment during a transition" for asking questions, if a story/task/issue is not clear enough. Specially used for supporters with low technical knowledge.
Is it possible to add the assignee field in the same screen? So that the commentator can ask someone specifically in one step? Otherwise the issue has to be edited again and the designated responder => assignee must be added manually in a further step...

Sure, in the same screen where the comments field is, you should add Assignee field too.

That´s correct. Comments field is one of the most used, but it’s not necessary to be, you can remove it if you want.
