I have 5 teams with 5 different boards. I would like a dashboard that would display all the teams names, their current active sprint names, their sprint goals, and maybe the epics contained in their active sprints.
Does any one know of an app, dashboard, widget, etc. that will display this information?

@Kevin Foster There is not really a dashboard that would incorporate everything you are asking for. Jira Align might work for your needs https://www.atlassian.com/software/jira/align There are apps in the marketplace that might meet your needs like BigPicture.
You can try ActivityTimeline plugin - it's a resource planning tool. There is a possibility to track users' tasks from different projects and teams, and see workload.

if I may recommend an app from the Atlassian Marketplace, I believe you would get pretty close - with not a lot of setup effort! - with the app that my team is working on, JXL for Jira.
JXL is a full-fledged spreadsheet/table view for your issues that allows viewing, inline-editing, sorting, and filtering by all your issue fields, much like you’d do in e.g. Excel or Google Sheets. It also comes with a number of advanced features, including the ability to group your issues by any issues field(s). With this, you can easily create a view like this:
This is grouping by team and active sprint, but you can group by any of your fields. You can also use different sum up techniques (or disable sum ups altogether), and use JXL's support for issue hierarchies and conditional formatting to build even more powerful views.

Does JXL for JIRA work across sites? We have only 5 teams but they are split across 2 sites.

oh, I didn't realise that you're working across multiple sites. JXL doesn't support that yet - it's definitely something we want to offer, but I can't commit to a concrete time frame yet. It sounds like Jira Align would be the way to go for you, unless you plan to consolidate your sites.

You are right, I should have mentioned we were using more than one site. We plan to consolidate but we don't have a firm date yet.
