I`m from the SaaSJet team and we have developed this add-on for collecting data, feedback, etc. Sharing the forms externally is also available.

As @John Funk and @Vishwas have said, this is not possible unless the person filling out the form has an account that is active in the destination Jira instance. I know from talking with Atlassian they are pondering the notion of "How could we make this possible", but it is not so easy....and many organisations security teams would not allow it even if possible.

Well that's not possible in Jira Work Management forms for unlicensed users.
For users to submit response in forms the have to be part of the userbase in jira. And they should have browse/create issue permission in that project.
