Issue Collector and Firefox 15/16

We have created a Custom Issue Collector in JIRA but only the Name and Email box appears when using that collector through Firefox 15 or 16. Is there any known issue? We have been able to successfully use IE8 as the browser, but many of our issue reporters will be using Firefox as their primary browser and I can't roll out to certain parties within my company if we can't get this to work with Firefox.

I can confirm this is happening with me as well. No Javascript errors. Very frustrating.
