Try using screen with comment and attachment field.

If you change the way of thinking about what you are trying to do. So instead of adding an attachment to a comment but think of it as adding a comment when you make an attachment you will find that this functionality is already present.

When you add the attachment JIRA provides you with the opportunity to add a comment.
