Jira icons appearing as attachments?

These attachments keep occurring on our issues.   I will delete them and they are still there or in duplicate or triplicate when I open it again.  Why is this happening?

I would suggest you to reach out to atlassian support if the issue is across the projects and issue types. Please add the resolution here so other can be benefited :)
