Multiple Issue Collectors On Same HTML Page Not Working

Good Afternoon,

We are running JIRA v6.3.13. I am trying to use four multiple issue collectors on one HTML page, but cannot get it to work. If you have this running on your site, can you please post a link so I can view the source code and figure what I did wrong?

I tried following the steps listed below, but I had no success. I cannot post the site I am working on because it is an internal site.
