I want to understand how JIRA would cope with the idea of a single instance with multiple clients? How would we set up the solution so clients only see the projects they should see but then have a project team, from an agency, that work across everything? Is it a massive admin undertaking or does it cope with it out of the box?

You can set up different projects with different workflow scheme for each client in jira.
You can use permission scheme at project level to determine who all can see this project or not.

Further to the response from Prakhar Srivastav you can share a permission scheme across all projects by defining a project role of client. Then each project lead can allocate clients on a project by project basis.
