Multiple tabs in a screen

Hello all,

I would like to ask if there is a way to add Jira system fields (assignee, reporter etc) in more than one tabs in a screen.
What I need is in one project if a user wants to create a ticket for Reason 1 or Reason 2 or Reason 3 to go to the tab with the name Reason 1 or Reason 2 or Reason 3 and fill in the fields.
However some of the fields need to be common between different tabs.
If you have any better ideas please advice.

One way to do that would be to use conditional logic on one form using ProForma.  The first question is a single choice question "What reason" - 1, 2, 3.  Depending on the answer they select, it shows the corresponding series of further questions.  eg:  Reason 1 is selected, then Reason 1 specific fields.

Sadly, fields cannot be placed in one more than tab at the same time..

Probably you can create a kind of dinamyc form by using Dynamic Forms for Jira third-party app.
