There is one single issue in our Jira project that has been created every day for the last 8 days and I am unable to determine why. It is an exact copy of the initial issue with the exact same history saying the same person created it.
I have checked that "Repeat Task" is turned off on all of them and reviewed automation.
Anyone have any idea of something else that could be causing this?
The screenshot says that there is an automation creating issues regularly. I think you'll need to take a closer look at your automations.
"Automation Request:" is text that is added to the issues as it is for our data team and they have requested anything logged with this specific issue type from our help desk have that added. It's not saying that it is being created by automation.

You note when you checked the history, it showed a specific person/user. That could still be from automation if the rule actor was changed.
Have you reviewed the automation rules/audit log in the project and at the instance level to see if there were any rule executions when these issues were created? That may narrow down the automation possibly doing this.
