I see from this link below that the 'common drivers' for creating multiple instances of Jira Software (cloud) does not include Dev/UAT/Prod environments. Does anyone have any experience of promoting creating that kind of structure and promoting configuration changes from one instance to another? Can you point me to any documentation that covers this?

I see also mention of Sandboxes...is this a viable use of a Sandbox attached to a production environment? Could I carry out config changes and export them to production? Again..if anyone can point to any blog or resource that might be useful?

In Cloud, sandboxes are indeed a good way to create a development environment. As you may have read in the article you mention, it is important to note that these are only available if you are on a premium or enterprise plan. And you can only have 1 sandbox per product that you own (e.g. Jira Software, Jira Service Management and Confluence).
As an alternative, it is very easy to create a free site on a trial license, but it's important to keep in mind that these trial licenses expire after 30 days. Through backup / restore you can pretty easily set up a copy of your production environment.
Promoting change from sandboxes or other sites are not very well supported as of today. While on server / DC tools like Configuration Manager (Botron/Appfire) and Project Configurator (Adaptavist) offered this capability, possibilities on cloud are limited.
Project Configurator was the first tool available to migrate (parts of) configurations between sites in the cloud. But the tool has just been acquired by Appfire as well, and the marketplace listing now refers to Configuration Manager as the tool to watch.
From the last information I know of, API capabilities of Atlassian Cloud product as of today do not allow the same possibilities of the on prem platform, but this is an area of constant change. So it is definitely worth keeping an eye on both the Configuration Manager roadmap as the Atlassian Cloud roadmap
