Creating work flow for service request

How to create service request to track life cycle which includes:

-requirements

-getting someone to approve

-go back to requirement

-finalize 

-requirement is then signed off

-design phase (high level and low level)

-build (including unit testing,system testing and integrating testing)

-user

-deployment

Also,how to create service request to know which phase the cycle is at
