Glad you found it! You can add fields using the "Add field" button (top-right) in your screenshot. This shows available fields.
If the field you need isn't listed, add it to the relevant screen first. Go to Project settings > Screens, click the relevant screen (e.g. "Approbation 1L Default Screen"), and add the field there. If the field's context is "Global", it should then be selectable in your form. If not, you'll need to configure the field context to include your service desk project.
For entirely new fields, create a Jira custom field and set the context to include your service desk project. This can be complex if you're new to Jira administration. Reach out to your Jira admin (whoever set up your service desk and ProForma) for guidance.

"Forms" in Jira can mean a few things.
If you're trying to edit the fields on a Jira issue screen (the one for creating/viewing issues), you can manage those in Project settings > Screens. You can add, remove, and rearrange fields there. For more guidance: Configure Issue Screens
If it's a JSM request form (the one that appear on a service desk portal) you can do this in Project Settings > Request Types. More guidance: Setting Up Request Types
ProForma forms are handled differently, here's their docs: Building Forms in ProForma

I suppose it is a Request type. But I don't find it in the project where it supposed to be. Is there any way research a Form by his name ? I need to change the Vendor Update but as you see, in the Proforma, there is none of it.

Thanks for the screenshots! It looks like "Vendor Update" is a Jira Service Management request type, not a ProForma form (since it's not showing in ProForma).
You mentioned it's not in the project where it's supposed to be. Double-check the project is called "Master Data" and try Project settings > Request Types. Also check similar service desk projects — portal and project names can differ. To verify you're in the right project, click "Raise a request" in the project's side menu — this opens the portal.
If it's the correct project but you still can't see the request types, it's likely a permissions issue. Check you are a project admin and a Jira Service Management Agent.
