I think we need clarification on this one. Could you please tell us more? Do you mean a JQL search, report or a custom field? What do you mean by "approve"?

As far as I understand you need a custom field in which you want to store the first comment made on the issue.

There is no built-in solution for this one. You have to write a script or use an app for this.

Enhancer Plugin is one of them. It has the Comment custom field which you can configure to display first/last one, and also you can filter by private/public options.

There might be other solutions as well, I am one of the folks behind this app and I will be happy to help if you need a further question.

Yep, if you define First comment custom field, you can use it in JQL of course.
