Before there was a button at the top that let me create an issue whenever I wanted. I needed to add a card to the current sprint. What I had to do in this new UI:
1. Go to the backlog
2. Scroll to the bottom of the page until I find a '+ Create Issue'
3. Write the title and create the issue
4. Move the issue to the sprint
5. Click on the issue to add a description
6. Click on another button to plan the time
(4-6 could be done in any order)
What I did before:
1. Click on the "Create" button and put all the information there in a single form
Is there a reason for removing the "Create" issue button?

In the new Cloud UI the create button has been replace by the plus sign on the left hand side. You can also use the keyboard shortcut "c" as well.

Once the sprint is started you do not have the Create issue, but I still see it for sprints that have not yet started.

I mean, do I need to open this second plan dialog every time? Or is there a way to configure the create dialog to include these fields as well?

That is different, that looks to be from Tempo. I do not have it in my Cloud environment, but we are using it in our Server environment and it looks similar. None of those fields can be added to the create screen as far as I know.

you are correct, the plan time dialog is a Tempo feature and can not be added to the Create issue screen.
It is only possible to add Tempo custom fields to the Create screen (the Tempo Team field and the Account field which are listed in the Custom fields list in JIRA settings, Issues, Custom fields).
